Max dose is 150 units per week. May exceed 24 weeks of usage based on goal progress.
Some people may not make it to the 150 units per week for a couple of reasons. One, perhaps
someone reaches their goal weight prior to reaching that dose, in which case we will then begin
to taper the dose back down with the hopes of completely tapering off the medication. Some
people may need to remain on a maintenance dose to maintain their weight loss goal. Two,
some people cannot tolerate higher doses. Perhaps someone has severe nausea or vomiting.
This is the body’s way of telling you that you can no longer increase your dose so then we will
find the right dose for you that is manageable, yet you are still losing weight. Please keep in
touch with how you are feeling as I want to ensure that you are having a good experience AND
success with the medication and treatment plan. The Dosing Instructions above are simply
guidelines.
I find that going up less works better. After week 4, if you are still losing weight, DO NOT GO UP
IN DOSAGE, stay where you are at. If you are not losing weight, go up to maybe 35 Units instead
of 50. Once you hit the max of 150 you cannot go any higher. You want to stay as low as
possible for as long as possible. This medication is only a tool not something to take advantage
of. Learn your eating habits as you go. Before you move up, ask yourself If you are doing
everything right. Are you eating enough protein, drinking enough water, getting enough sleep,
etc. Tweak your routine a little to see if that helps before moving up. Sometimes a little
adjustment is all it takes.
MEASUREMENTS: I suggest always taking your measurements before starting and at least once
every 3 months. There will be a lot of non-scale victories in this journey and you will have
frustrations. The measurements will assure you that you are indeed on the right path and you
will be surprised at how much you are not only losing in weight but in inches as well. It is not all
about the weight loss but the inches that happen. Take those photos!!! You will regret it if you
don’t.
Dosing: Please remember to only increase your dose if your current dose is no longer effective
and no side effects are present.
Please note: There are several different concentrations depending on the pharmacy. ALWAYS
check the directions with each new order you receive. The concentration of the vial can differ
which changes the amount of units needed to achieve a particular dose. If you need
assistance with your dosing, please let me know.
Recommendations for dosing adjustments
Keep in mind that since everyone is different, the dosing schedule I provide is sometimes not
right for everyone
Signs your current dose is working well:
1. Good appetite suppression
2. Steady weight loss (1-1.5 pounds per week)
-Note: I recommend staying on that dose until it is no longer working for you. It is not
necessary to increase the dose if it works. If you increase your dose when its not
needed, side effects are more likely to occur as well. If you are having good appetite
suppression, weight loss typically follows.
Signs it may be time to increase your dose:
1. Weight loss has completely stopped for 2-3 weeks or more
2. Hunger is back a few days after the injection day (note: it is normal and healthy to be
more hungry the day before your injection is due)
-Note: if you have only been on a dose for 1-2 weeks, I recommend only increasing a
small amount (ex: 5-10 units). Please ask me about your specific dose increase
recommendation. I may have other options for you.
3. If you have taken a GLP1 or GLP2 in the past and started at a lower dose, it is possible
you could have a tolerance to those lower doses. The lower doses may not have the
same effect on appetite suppression and weight loss as they did before. In this case,
increasing your dose should help if you are having a stall in weight loss.
DO NOT increase your dose if the following is occurring:
1. Experiencing side effects- do not increase your dose until side effects are gone. Please
always notify me of any side effects to determine if decreasing the dose or holding a
dose is necessary
2. Forcing yourself to eat- we do not want you to get malnourished. If your body thinks its
starving, it will start to also hold on to weight and you will stop losing.
3. Losing more than 2 pounds per week (Losing weight too quickly can lead to other issues
we want to avoid)
Education: Before starting the program, I recommend reviewing the information provided,
understand diet, exercise and that you do need to drink water and eat lots of protein. You do
need to put in some work in order for this to work. This is a lifestyle change.
Injection Videos: DO NOT guess on how to do injections. Please reach out to me if you are
second guessing how to do the injections yourself and I am happy to show you or always look
up how to do them on You Tube.
Side Effect Management: Do not suffer! I have Zofran if you feel nauseous. I am happy to give
you some nausea pills to help.
(Please note: side effects should be mild and only last 1-2 days after injection. If your side
effects are lasting all week or you are experiencing vomiting/ abdominal pain/ constant
diarrhea: hold your next dose and notify me or your own dr. immediately.)
CONSULT WITH YOUR OWN PROVIDER: I cannot emphasize enough that I am not a provider

medication does not work for everyone. If you are taking medications there are some that will
counteract with Tirzepatide and actually make you gain weight instead of lose. This can not
only become frustrating but I need to know if you are taking any medications so we can make

where your provider can consult with you ahead of time to see if this is a good fit for you or
